Job Responsibilities
- Assessment: Conducting thorough assessments of the patient's health condition, including physical, mental, emotional, and social aspects. This involves monitoring vital signs, evaluating symptoms, and assessing the patient's overall wellbeing.
Medication Management:
Administering medications as prescribed by the physician, ensuring proper dosage and timing. They also educate patients and their families about medication usage and potential side effects.
Wound Care:
Providing specialised care for wounds, including cleaning, dressing changes, and monitoring for signs of infection. They may also teach patients and caregivers how to manage wound care between visits.
Personal Care:
Assisting patients with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, and mobility. They may also help with feeding and monitoring dietary intake.
Patient Education:
Educating patients and their families about their health conditions, treatment plans, and self-care techniques. This includes teaching them how to manage chronic conditions, prevent complications, and promote overall wellness.
Monitoring and Reporting:
Regularly monitoring the patient's health status and reporting any changes or concerns to the healthcare team. They maintain detailed records of patient assessments, interventions, and progress.
Emotional Support:
Providing emotional support and reassurance to patients and their families, especially during challenging times such as illness or recovery from surgery.
Safety:
Ensuring a safe environment for the patient by identifying and addressing potential hazards in the home. This may involve recommending modifications or assistive devices to improve safety and independence.
Coordination of Care:
Collaborating with other healthcare professionals involved in the patient's care, such as physicians, therapists, social workers, and home health aides.
They help coordinate appointments, treatments, and services to ensure continuity of care.Advocacy:
Advocating for the patient's rights and needs, including ensuring they receive appropriate medical care, support services, and community resources
