    • Ensuresplan is reflective of admission or outpatient database, on-goingfindings, age appropriate care, cultural specific needs, andappropriate acuity.
    • Ensures plan is discussedwith patient, family/significant others, and completed in a timelymanner.
    • Monitors the patients progress based onthe plan.
    • Revises plan on ongoing basis basedon patient condition and evaluation ofprogress.
    • Ensures care meets standards ofpractice.
    • Ensures effective development andcompletion of discharge plan including discharge barriers andpatient/family education.
    • Ensures that patientclearly understands discharge instructions. In outpatient, identifybarriers and needs for patient/family education that willfacilitate the outpatient medical managementplan.
    • Ensures patient safety related but notlimited to, medications and procedures utilizing the five rights;patient falls; decubitus prevention and prevention of nosocomialinfections.
    • Ensures optimal pain control andpatient comfort; identifies and discusses patient anxieties, fearsor concerns regarding patient condition, treatment ordischarge.
    • Ensures that patient understandsmedication purpose, side effects, and administration instructionsin the hospital as well as at the time ofdischarge.
    • Patient CareExperience:
    • Practices customer servicestandards as defined by the Service Area, Medical Center, andspecified department.
    • Promptly answers calllights, alarms, and patient requests.
    • Makesappropriate referrals and facilitates the customers ability toutilize resources.
