    Description

    Job purpose:

    The Staff Nurse (SN) II is a professional caregiver who assumes responsibility and accountability for a group of patients for a designated time frame. The SN II provides care to patients via the therapeutic use of self, the nursing process, the environment and instrumentation, and other health care team members.

    Essential Duties and Responsibilities:

    • Provides and monitor direct and indirect patient care to achieve optimum patient outcome.
    • Assumes accountability and responsibility for own practice and does not carry out tasks beyond level of competence.
    • Incorporates the religious and cultural needs of the patient in their individualized care and includes input from family members.
    • Coordinates individualized care from point of first contact throughout the episode of care, incorporating discharge planning.
    • Maintaining timely and accurate nursing records and ensure that confidentiality is respected.
    • Maintaining personal contact with patients and relative being sensitive to their needs for courtesy, dignity, privacy and always ensuring friendly environment.
    • Independently evaluates, plans, and implements individualized care plans for patients and assists junior staff with this process.
    • Documents all assessments, interventions, and other pertinent information in the patient's health care record in a timely and ongoing manner.
    • Aware of the role of audit and research, can maintain an effective audit trail and work towards continual improvement in quality assurance program.
    • Demonstrates service excellence and acts as a patient advocate to always support the patient's rights and dignity.
    • Ensures all equipment and resources are maintained and used, economically and efficiently.
    • Promptly reports to Nurse Manager Unit/ Charge nurse/ Team Leader and assists in investigating complaints and incidents in relation to patient care in accordance with Hospital Policy.
    • Coordinates referrals and utilizes available resources for the provision of comprehensive care to patients and families.
    • Perform antenatal training for the expected mother and family.
    • Promotes and utilizes evidence-based practice in the provision of care.
    • Facilitates a climate of mutual understanding, and appreciation of cross-cultural differences inherent in a multicultural society.
    • Administers medications according to organization policies and procedures.
    • Understands actions of medication and contraindications, evaluates patient's response, identifies side effects, and initiates appropriate action.
    • Report adverse Drug reactions and medication errors as per DOH/DHA regulations.
    • Ensure the storage and administration of narcotic and controlled drugs is in accordance with DOH/DHA Narcotic policies and procedures as per the area requirements.
    • Maintains a sound knowledge base in medication and administration of medications.
    • Responds to all levels of emergencies according to the organization's policies and procedures.
    • Demonstrates knowledge of and applies safety principles as identified within the institution.
    • Provides data for staffing decisions and demonstrates flexibility in the resolution of staffing issues; demonstrates flexibility when floating to other units upon requests.
    • Provides guidance and advice to the patient care team on clinical and administrative matters.
    • Performs accurate and complete admission assessments.
    • Create individualized care plans for a patient under their supervision and care.
    • Collaborate with a team of healthcare professionals to share expertise and knowledge.
    • Give supportive resources to the patient and the families to promote a positive outlook.
    • Teach patients and their families regarding treatment expectations.
    • Watch and record the patient's progress constantly.
    • Extend service within their scope of practice and competency whenever floated to another department.
    • Chart the patient's response to treatment and medication.
    • Ensure to comply with the ADHICS standards and responsibilities.
    • Always maintain and ensure Patient Health Information confidentiality with regard to IT/ Other aspects.
    • Not to release any information/data/ records to anyone without the knowledge and approval of the concerned Supervisor/ HOD's.
    • Responsible for maintaining confidentiality in all matters according to ADHICS/ DOH/DHA/ JCI Policy.
    • Participates in nursing research activities as requested.
    • Perform other related duties as necessary.
