- Interprets physician prescriptionsand medication orders. Acts as a drug information resource topatients, medical staff, nursing staff and ancillary departmentpersonnel.
- Review medication orders, clinicalappropriateness and dosage adjustments, interchanges and therapymodifications as appropriate as per policy before dispensing forthe individual patient.
- Conduct appropriatenessreview for each medication order (Drug, dose, frequency and routeof administration, Duplication, Allergies or sensitivities,Interactions, Variation from hospital criteria for use,Patient's weight andcontraindications).
- Medicationreconciliation.
- Timely and accurate dispensingof prescriptions to inpatients and dischargepatients.
- Identify and resolve medicationrelated problems with the prescriber before processing themedication order.
- Appropriate labeling ofmedications to ensure proper patient identification and safemedication use
- Follows and maintainsestablished procedures concerning High Alert Medications, Controlmedications, disposal of hazardous waste and comply with hospitalpolicies.
- Counsels patients or carers onprescription to ensure that the patient understands all informationrequired for safe and proper use of medication and storage athome.
- Promotes rational, cost-effective drugtherapy
- Compounds medications according toHospital guidelines and procedures.
- IncreasesPharmacy-Physician interaction by providing an immediate source ofinformation as well as encouraging the use of the hospitalformulary of the approved drugs
- Providesmedication information to nurses, patients and other healthcareprofessionals.
- Participates in quality use ofmedicines projects, audits and dispensary based quality assuranceactivities. Arranges and processes ward stock replenishment ofmedications.
- Assists and participate ineducational programs (departmental, medical, nursing,community)
- Ensures the maintenance and properoperation of equipments and supplies in the workarea.
- Assists the Chief Pharmacist on specialassignments.
- Demonstrates ability to interpretan extensive variety of technical instructions and deal withseveral abstract variables.
- Follows insurancecompany's policies andprocedure.
