ESSENTIAL DUTIES & RESPONSIBILITIES:


  • Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative long term nursing actions. Includes the patient and the family in the planning process.
  • Actively contributes to Care Plan development: assessment, evaluation and redevelopment
  • Review and updates as needed the comprehensive assessment, clinical notes, Physician Summaries, Plan of Care and communicates patient status changes and updates to the primary physician regularly.
  • Communicates with the duty physician regarding the patient's needs and reports any changes in the patient's condition; obtains/receives physician's orders as required.
  • Uses health assessment data to determine nursing diagnosis
  • Follows all International Patient Safety goals including the medication rights to reduce the potential for medication errors.
  • Monitors the patient's progress toward goals; collaborates with the multi disciplinary team to promote patient safety and evaluates against the Plan of Care.
  • Initiates appropriate preventive and clinical nursing procedures. Administers medications and treatments as prescribed by the physician.
  • Counsels the patient and family in meeting nursing and related needs.

Education & Research:


  • Works with all healthcare team members in activities designed for the improvement of patient care outcomes
  • Acts as a preceptor and/or resource for new employees, trainees and other staff
  • Assumes responsibility for own professional growth identifying those areas of clinical knowledge and skills that need further development
  • Uses research/current references in the provision of evidencebased care
  • Participates in teaching and mentoring of Nursing

  • Assesses own care, that provided by others, patient care systems, and continuously seeks ways to improve

Compiles with and participates in Quality Management Processes and activities

  • Performing general nursing services, checking vitals and general health assessments.
  • The diagnosis, assessment, planning, implementation and evaluation of the patient's care.

REQUIRED PROFILE

  • A Bachelor degree in Nursing/ Healthcare
  • Valid MOH license or at least Eligibility letter
  • Experience in working within a Clinic/Hospital/Home Care settings

SKILLS & EXPERIENCE

. Minimum 2 years of experience
