    Description

    PositionSummary

    This position assesses,plans, implements, evaluates and documents nursing care of patientsin accordance with organizational policies and in accordance withstandards of professional nursing practice utilizing the frameworkfor professional nursing practice and development. This position isaccountable for the quality of nursing services delivered by selfor others who are under their direction. This position utilizesspecialized knowledge, judgment, and nursing skills necessary toassess data and plan, provide and evaluate care appropriate to thephysical and developmental age of assignedpatients.

    CoreFunctions

    • Assesses patient physical,psychological, social, spiritual, educational, developmental,cultural and discharge planning needs. Reviews patient history andphysical with patient/family and assures completion withinappropriate timeframe. Reviews available information obtained byother health care team members. Reviews diagnostics and laboratorydata and reports abnormal results to the physician(s) and otherappropriate caregivers. Completes assessment and reassessmentsaccording to patient need and as outlined inpolicy.
    • Formulates a plan of care, includingthe discharge plan, utilizing assessment data and patient, familyand health team input. Initiates a plan of care based onpatient-specific needs, assessment data and the medical plan ofcare. Goals for patient are realistic, measurable and developed inconjunction with the patient/family. Considers the physical,cultural, psychosocial, spiritual, age specific and educationalneeds of the patient in the plan of are. Plans care incollaboration with members of the multidisciplinary team. Reviewsand revises the plan of care to reflect changing patient needsbased on evaluation of the patient'sstatus.
    • Implements the plan of care throughdirect patient care, coordination, delegation and supervision ofthe activities of the health care team. Provides care based onphysician orders and the nursing plan of care, in compliance withpolicies and procedures, standards of care, and regulatory agencyrequirements. Delegates appropriately, and provides nursingsupervision in the provision of care to patients by other licensednurses and other personnel. Promotes continuity of care byaccurately and completely communicating to other caregivers thestatus of patients for whom care isprovided.
    • Evaluates the patient's andfamily's response to care and teaching, and revises theplan of care as needed. Evaluates patient progress towards goalsand expected outcomes in collaboration with other health care teammembers. Evaluates patient's response and the effectivenessof patient teaching.
    • Documents assessment,planning, implementation and evaluation in the patient record.Documentation is legible, timely and in accordance with policy.Documentation reflects objective/subjective data, nursinginterventions and patient's response to treatment. Notesphysician orders accurately and in a timelymanner.
    • Provides care based on the bestevidence available and may participate in research activitieswithin clinical practice. Participates in unit or facility sharedleadership. Interacts and participates in the education, roledevelopment, and orientation of facility personnel, patients,students, families and visitors. Promotes/supports growth of othersthrough precepting and mentoring whenappropriate.
