    Position:PatientSupportProgram Nutritionist

    • The PatientSupportProgram Nutritionist provides comprehensive Patient care in the community setting that assists patients to achieve optimum health and quality of life in situations of chronic disease. The Patient Support Nutritionist is responsible for the provision and management of effective Patient care to individuals and/or families in the community or in-Patient Support Programs. The Patient Support Nutritionist works in adherence to the Policies and Procedures set by Axios International and works in accordance with established standards of Patient practices according to local and international rules and regulations.
    • Duties and Responsibilities
      • Telephonic follow-up consultation
      • Ensures clients' menu selections are in compliance with prescribed diet order, nutritional adequacy, age, cultural background, and religious practice.
      • Writes and modifies menus as needed according to client diet prescription using established resources/guidelines.
      • Updates, monitors, and follows-up on all pertinent client information like diet order changes, food allergies, food preferences, and requests.
      • Records all assigned data such as client meal census and floor stock requests on paper logs or spreadsheet.
      • Resolves client requests and complaints or relays to appropriate staff.
      • Directs members and staff of Food and Nutrition Services office.
      • Serves as a dietary resource for nursing and other staff.
      • Provides general information on diet utilizing approved materials and resources.
      • Education and counselling, consistent with the type/nature of treatment and/or care management need.
      • Providing general Patient care.
      • Supervision/health teaching and support for family and caregivers on regular basis.
      • Attending and completing Pharmacovigilance, Pharma and Product Training provided by the Client with a clear understanding of how to collect and report safety data.
      • Reporting any adverse events/product complaints as per client's requirements and/or Axios SOPPs.
      • Conduct all program activities through exclusive use of Axios' Patient Management System (PMS).
      • Additional duties as assigned.
    • Job Circumstances
      • The position is based in our regional office, and it does entail frequent travel to Clients, partners and Pharmacies which may be outside your designated region.
      • Flexibility in ability to travel as well as working hours is essential.
