    CoreDuties andResponsibilities:

    • SupportingPatientCare:
      • Facilitatecare delivery to patients.
      • Coordinateoperations within the medicalcenter.
      • Communicate and provide resources topatients and caregivers regarding conditions and treatmentrequirements.
    • BehaviorManagement:
      • Recognizeand manage challenging behaviors of patients and caregiverseffectively.
    • CommunicationandLiaison:
      • Liaisewith patients health and social care providers for ongoingcommunication.
      • Organize patient information forefficient datatransfer.
    • PatientInformationHandling:
      • Receiveand collate information from new existing and transferpatients.
      • Ensure adequate monitoring forpatients at risk by liaising with otherproviders.
    • DataQuality andReporting:
      • Conductchecks on data input for quality andconsistency.
      • Generate reports for populationhealthmanagement.
    • MultidisciplinaryCoordination:
      • Coordinateamong multidisciplinary teams within the providerssetting.
    • InternalMeetings andMinutes:
      • Organizeand manage minutes of internal meetings asrequired.
    • FollowupandTracking:
      • Followup and track preventive screening results to ensure timeliness andcompleteness.

    • Clinicallead.
    • Physicians within thefacility.
    • Administrative staff within thefacility.
    • Other clinically trained personnelsuch as pharmacists technicians physiotherapists nursesetc.

    • Service providers likerehabilitation centers home care facilities dialysis centersoutpatient pharmacies etc.
    • Social care servicesprovided by psychologists behavioral analysts social workerscounselors etc.
    • Voluntaryservices.
    • Patients and their familymembers/caregivers.

    PreferredQualifications:

    • Diplomaor equivalent certificate in Healthrelated fields (e.g.Health Care Administration Public Health Health Education) SocialSciences or BusinessAdministration.
    • Bachelors degree inHealthrelated fields (e.g. Health Care AdministrationPublic Health Health Education) Social Sciences or BusinessAdministration.

    PreferredExperience:

    • Minimum 2years of experience at a healthcare provider (e.g. hospitaloutpatient clinic) or with a healthcareprofessional.
    • Medical center operation andcoordination experience.
    • Quality experience inJAWDA TASNEEFKPIs.
    • Administrative duties anddatabase usage experience.
    • Experience inprimary care services (such as chronic disease management andpreventive services).
    • Knowledge of medicalterminology.

    Note:Experience requirements are flexible if appropriate education andtraining are met.
