• Informing the Permit Holder of thework to be done, the exact location of the tools and plant/equipment to be used in the work, the hazards that may be presentand all the precautions prescribed on the Work Permit that must betaken during the work and ensuring the Permit Holder hastransmitted those instructions to his staff andContractors
• Identifying and addressing anyconflicts between the proposed work and other activities in thearea, and if necessary cross-referencing the relevant Work Permitsin the location
• Confirming that all hazardsassociated with the work have been identified during the workpreparation process and all relevant parties have provided theirinput and such parties agree with the precautions to be takenduring the work activities, e.g. throughsupporting
• Ensuring that all legal requirementsrelevant to the work have been fulfilled prior to issue of the WorkPermit and continue to be satisfied during the carrying out of thework subject to the Work Permit. Ensuring that all necessarysupporting Certificates, Job Hazard Analysis and drawings areattached to the Work Permit
• Confirming that alloperational preparations (including process clearance andisolations) have been completed.
• Confirming,prior to commencement of the work, that the conditions in respectof the worksite have been met and therefore it is safe for the workto proceed.
• Describing in the Work Permit whatprecautions need to be taken during the work, including use ofsafety equipment and Personal Protective Equipment(PPE).
• Notifying all people who may be affectedby the work covered by the PTW System that the work Permit has beengranted.
• Signing and issuing the Work Permit tothe Permit Holder.
• Displaying the status of thework at all times (e.g. on a display board) and communicating thestatus to the oncoming shift at shifthandover.
• Signing off and cancelling the WorkPermit when the work is complete, abandoned orsuspended.
