ProgramCoordination

Servesas a member of the leadership team, including the ResourceNavigation Manager, Community Support Team Manager, Street TeamManager, Data and Communications Manager, and Hospital BasedInterventionist in order to ensure a comprehensive plan for theentirefamily
Incoordination with the hospital-based interventionist and ResourceNavigation Manager, ensure families receive an assignment to aFamily SupportSpecialist
Ensurereferrals to intensive wrap-around partner services for eligibleyoungersiblings

StaffSupport

AlongsideHR Director, coordinate and facilitate hiring, onboarding,conducting regular evaluations, coaching, and ongoingtraining
Setclear, attainable goals with Family Support Specialists and holdperformance reviews, encouraging learning and educationalopportunities, both professionally andpersonally
Conductconsistent (weekly or bi-monthly) one-on-one supervisions withFamily Support Specialists to review participant service plans,barriers to attaining goals, and successes as well as professionaldevelopment opportunities forstaff
Ensurecase notes, strength and needs assessments, and other files arecompleted by Family SupportSpecialists
Identifypatterns in data collection to Contract Manager for follow uptrainings for data integrity
Facilitateweekly team meetings and ensure notes aretaken
Coordinateoverall staffing plans and program leaves or vacation requestsensuring appropriate coverage forprogram

Familyand SiblingSupport

Workin coordination with the Resource Navigation Manager and FamilySupport Specialists to support families impacted by gun violence
Maintainrapport with participants and their families, as well asparticipate in meetings with families and Family SupportSpecialists asneeded
Facilitatede-escalation and conflict resolution for families as needed
Viastaff supervision, ensure family linkages to needed support andservices
Serveas a point of contact for young people, impacted families, andhospital based interventionists for any questions or risingconcerns

ProgramManagement

Steerthe implementation of best practices as identified in coordinationwith ExecutiveLeadership
Workwith other Program Managers within CP to collaborate on bestpractices and identify means of collaboration as appropriate
Overseeprogram deliverables andresults
Ensurecompliance to regulatory and funderrequirement
Leadprogrammatic visioning and implementation of Align services andprogramming with community needs, client needs, and funder/contractexpectations
Submiton-time accurate reports required by funders and the Attend allfunder and agency-required meetings and represent CommunityPassageways on panels, committees, and workgroups asneeded
Reportto Executive Leadership to ensure program budget, data, and servicecompliance
