    Description

    Responsibilities:

    • Provides home visits topregnant/parenting people and their families eligible for the NFPProgram.
    • Provides education and case managementservices to people primarily in Far Rockaway, Queens from pregnancythrough the child's secondbirthday.
    • Adheres to nursing process and theNFP model of home visitation.
    • Carries acaseload of a minimum of 25 clients.

    ClinicalDuties:

    • Develops therapeutic relationships,utilizes concepts of reflection and motivational interviewing withpregnant people and their families in a home visitingenvironment.
    • Performs home visiting inaccordance with the NFP model andguidelines.
    • Follows nursing process inimplementing NFP into nursingpractice.
    • Assesses physical, emotional, socialand environmental needs of pregnant people and their families asthey relate to the NFP domains.
    • Assistspregnant people and their families in establishing goals andoutcomes.
    • Provides education, support andreferral resources in assisting pregnant people and their familiesin attaining their targeted goals.
    • Consults andcollaborates with other professionals involved in providingservices to pregnant people andfamilies.
    • Formulates nursing diagnosis based onnursing assessment and client goals.
    • Refersfamilies for necessary services and following up to ensure thatfamilies are receiving services.
    • Completes alldocumentation required for Medicaidbilling.
    • Performs related duties as assigned orrequired.

    Qualifications andSkills:

    • BSNrequired or associates if currently enrolled in a BSNprogram.
    • Current New York State RN license (ingood standing) required.
    • Current CPRrequired.
    • Bilingual English/Spanishrequired.
    • Two years recent experience inmaternal/child health, public health, home visiting ormental/behavioral nursing preferred.
    • Homevisiting experience preferred.
    • Excellentwritten and verbal communication skills.
    • Basiccomputer skills.
    • Valid driver'slicense and insured automobile may berequired.
    • Must be able to work a flexibleschedule including some evenings andweekends.
    • Salary:$85,000.00-$85,000.00.
