    Description

    Creates new medical records and retrievesexisting medical records by gathering appropriate record foldersand contents; assigning and recording new record numbers; verifyingexisting record numbers; inputting and recording locations tocomputer; delivering records assembling and verifying content andmatching loose reports to correct patient records.

    PositionResponsibility:


    •Maintainsrecord availability by processing charts in the department; usingchart mark-off procedures; and facilitating chart locationactivities.


    •Retrieves medical recordsby following chart-out procedures; documenting reasons chartscannot be retrieved for statistical and follow-up purposes.


    •Delivers charts to assigned areas ofthe hospital by following established routingprocedures.


    •Keeps health care providersinformed by communicating the availability or unavailability of therecord.


    •Maintains quality results byfollowing hospital standards andpolicies.


    •Maintains continuity of workoperations by documenting and communicating actions,irregularities, and continuingneeds.


    •Enhances medical records andhospital reputation by accepting ownership for accomplishing newand different requests; and exploring opportunities to add value tojob accomplishments.


    •Initiates themedical record by creating and processing the patient care recordfolder as per the content list.


    •Matches all loose reports using atleast two patient identifiers.


    •Preparesthe records according to the operating room schedule and deliversthese records to the appropriatewards.


    •Filings of patient records usingthe approved filing system.


    •MaintainsPurging and Archiveworksheet and ensures timely and as-per-policy purging,offsite storage, recall andarchive.


    •Collects, assembles, and/orfiles into sorter loose reports andinformation.


    •Maintains confidentialityof all data and information at all times.


    •Performs additional duties assigned toMedical Records Coordinators such as assembly, and analysis duringpeak times and leave coverage.


    •Forbbids of using copies, sharedfolders, and external devices during any management of informationdata.


    •Ensures that records are kept upto date and that any necessary changes or additions are made in atimely manner.


    •Prints pull list andtracer cards of booked clinic appointments prints clinic add-onlist and prepares patient records accordingly.


    •Assembles of new patient records asper the approved index.


    •Prepares shadowpsychology records.


    •Ensures theappropriate usage of the chart tracking module ensuring thatpatient record locations are continuouslyupdated.


    •Tags any incomplete recordsprior to submission tocoordinators.

    PositionQualification:


    •HighSchool Diploma

    PROFESSIONALEXPERIENCE:


    •Experience arecommendation (Medical Records / HealthInformatics)


    •Knowledgeable aboutmedical terminology, appropriate level of healthcare and healthcaredelivery systems would be abenefit


    •Advanced Computer Skills inWindows, MS Office, Excel, PowerPoint, Internet andEmail


    •Ability and comprehension inwritten and spoken English


    •Knowledge ofterminal digit filing an asset
