    JobDescription:

    • Supportcare delivery to patients
    • Medical CentreOperation coordination
    • Communicate and provideresources to patients and caregivers about patients conditions andtreatment requirements
    • Recognize andappropriately manage challenging behaviors of patients andcaregivers
    • Liaise and follow through withpatients and all health and social care providers to keep everyoneinformed and updated
    • Organize patientinformation for efficient data transfer to relevant internal staffand external providers
    • Receive and collateinformation from new existing and transferpatients
    • Liaise with other providers to ensurepatients at risk are monitoredadequately
    • Conduct checks on data input toensure quality and consistency
    • Generate reportsfor population health management
    • Coordinateamong multidisciplinary teams in the providerssetting
    • Liaise with clinical and nonclinicalstaff for effective patient management
    • Organizeand manage minutes of internal meetings asneeded
    • Followup and tracking of preventivescreening results to ensure the timeliness and completeness offollowup as specifications andcarepathway

    Qualifications:

    PreferredQualifications:

    • Diplomaor equivalent certificate in Healthrelated fields (e.g. Health CareAdministration Public Health Health Education) Social Sciences orBusiness Administration
    • Bachelors degree inHealthrelated fields (e.g. Health Care Administration Public HealthHealth Education) Social Sciences or BusinessAdministration

    PreferredExperience:

    • Minimum 2years of experience at a healthcare provider (e.g. hospitaloutpatient clinic) or with a healthcareprofessional
    • Medical centeroperation andcoordination
    • Qualityexperience in JAWDA TASNEEFKPIs
    • Experience inadministrative duties and use ofdatabases
    • Experience in primary care services(such as chronic disease management and preventiveservices)
    • Knowledge of medicalterminology

    Skill SetandAttributes:

    • Excellentverbal and written communication skills in English andArabic
    • Strong organizational and documentationskills and attention to detail
    • Strong criticalthinking and problemsolving skills
    • Proficiencyin the use of common word processing presentation spreadsheet andemail tools
