    Description

    EssentialResponsibilities

    • Provides nursing interventionsrequiring substantial and specialized nursing skill, and initiatesappropriate preventive and rehabilitative nursing procedures.Follows through the plan of care as developed to: provideappropriate instruction to patient/caregiver about health care,disease management and adjustment to illness; direct and supervisethe care and services of LPN and HHA in the home setting; performtreatments; instruct patients/families in self care activities andhome safety; and competently perform skills/tasks per plan of care.This includes pain and symptom management in the home. Performsskilled procedures to include injections, dressings, catheterinsertion and care, colostomy and tracheotomy, central line IV careand blood draws. Identifies educational needs. Instructs patientaccording to the plan of care. Provides care and case managementwith interdisciplinary team.
    • Communicates andcollaborates with other interdisciplinary team members and with thepatient family in planning and implementing care. Records accurateand pertinent data in the electronic medical record in a timelymanner, according to documentation guidelines. Adheres to KFHPWinfection control, safety, hazardous waste, universal precautions,and CPR standards.
    • Identifies own learningneeds, including those related to age-specific populations andparticipates in educational efforts to meet those needs.Participates in the following: staff orientation; self andcolleague/team performance reviews; and activities related toquality care and cost saving measures. Adheres to KFHPW personnelpolicies and maintains a high standard of professional work habits.Demonstrates an appearance consistent with KFHPW dress codepolicy.
    • Assesses patient need for care andcommunicates with other health care providers appropriately and ina timely manner. Collects data relevant to visit throughout theduration of care. Develops and initiates the plan of care based onassessment. Communicates and collaborates with medical doctor(s) toestablish plan of care.
    • Evaluates patientresponse to care through timely assessment, communication andobservation. Modifies plan of care through assessment based uponpatient and care systems response.

    Experience

    BasicQualifications:

    • Minimum two (2) years of experienceas Registered Nurse in medical surgical, home health orhospice.
    • Per the National Agreement, current KPCoalition employees have this experience requirementwaived.

    Education

    • Associates degree in Nursing from anaccredited program. High School Diploma OR General EducationDevelopment (GED) required.

    License, Certification,Registration

    • Driver's License (Washington)required at hire
    • Registered Nurse License(Washington) required at hire
    • Basic LifeSupport required at hire

    AdditionalRequirements

    • Proof of automobileinsurance.
    • Demonstrated experience providingcare planning within a community setting (continuum ofcare).
    • Ability to workindependently.
    • Ability to transfer/positionpatients as needs require.
    • Basic laptop skillsincluding MS Office Suite.
    • Customer serviceskills.

    PreferredQualifications

    • Horizon for homecare systemexperience.
    • Bachelors degree in Nursing orrelated health care field.
