    Description

    EssentialResponsibilities

    • Reviews and prepares medical recordsfor assigned patients; initiates follow-up for medical record asrequested; reviews patients needs; identifies and prepares ordersfor needs of patient for authorization by practitioner and mayaccept verbal orders per policy; observes, collects, and documentsnew information on patient; informs RN or practitioners regardingincomplete data, new observation or abnormal/unusualdata.
    • Implements basic nursing procedures perplan of care; administers treatments, immunizations, andmedications as prescribed by practitioner; prepares medications forpractitioners to use; performs point of care testing as authorized;provides laboratory, x-ray, and other reports to practitioner forreview; notifies patients of normal results as directed bypractitioner; considers special needs of different age groups whenproviding care; sets priorities of care based on acuity ofpatients.
    • Documents implementation oforders/interventions; records findings, instructions, and patientresponses on appropriate forms or on-line records using departmentand/or JCAHO standards.Observes patients/familys responses to planof care; reports unexpected responses to RN or practitioners;collaborates with RN/Practitioner to revise care plan when needed.Demonstrates and applies competency in basic nursing skills and useof equipment; maintains current knowledge and skills needed toperform position; attends and participates in staff meetings asneeded; participates in projects asrequested.
    • Performs efficiently in emergencypatient care/clinic situation; follows established procedures;informs appropriate practitioner/RN. Assigns basic nursing tasks tounlicensed personnel as appropriate and according todepartment/unit standards; provides clear directions; acts asresource person to unlicensed staff; follows-up on assigned care toensure it is given per standards, policies, procedures, protocols,and guidelines.
    • Follows discharge plandeveloped by Practitioner or RN; processes and/or follows-up withreferrals as directed by practitioners; provides instruction andpre-printed information to patients as directed by RN and/orPractitioners, observes and documents patient/family response toinstructions and reports any concerns.
    • Managesall patient vaccine data, including updating Hawaii ImmunizationRegistry System (HIR), Electronic Health Records (I), and allpatient and/or external records at each patient visit. Readies anyavailable immunization information prior to the time of visit withthe provider. Stores and administers immunizations according toHealth plan and VFC guidelines.
    • Identifiespatient immunization requirements in accordance with age specificguidelines per CDC. Fills out appropriate immunization paperworkfor recording purposes. Facilitates vaccine Health Plan and VFCinventory control at assigned clinic. Ordering vaccine andmaintaining appropriate vaccineinventories.
    • Utilizes file maintenance toensure newly arrived vaccines are entered in the I lot managersystem as needed. Assures immunization clinics comply per healthplan, federal and state guidelines - i.e., compliance with Healthdepartment site visits. Complete annual VFC enrollment trainingrequirements and forms. Maintain all records related to the VFCprogram per program guidelines.
    • Trains new andexisting LPNs on emergency vaccine storage procedures.Serves as aliaison with the state to coordinate VFC (vaccines for children)inventory. Generates reports on patients and contacts them forimmunization compliance and updates asneeded.
    • Assists in ensuring compliance withinfection control policies and protocols and follows medicationadministration, storage procedures, Health plan CDC, State andfederal VFC program guidelines. Provides educational materials topatients and reviews printed instructions with patients;collaborates with RN/Practitioner to revise teaching plan whenneeded.
    • Participates in orientation of newpersonnel. Demonstrates sufficient knowledge, skills, and abilitiesin area of specialization. Performs clean/sterile technique, setsup for procedures, assists with clinical procedure withindepartment scope. Follow the recommended process for handling pointof use instruments as outlined by the High-Level DisinfectionCommittee.
    • Maintain, order, organize, rotate,and stock supplies. Schedule patients for routine care, acute care,and addressing care gaps. Prepares and processes patient forms.
    • OHS only: Certification in Spirometry,Hearing Conservation, & Substance Abuse Specimen Collectionor acquire certification within one year ofhire.
    • Demonstrates knowledge, skills, andabilities necessary to provide care and/or service appropriate toage groups served.
    • Demonstrates knowledge,skills, and abilities necessary to provide culturally sensitivecare and/or service.
    • Performs other duties andaccepts responsibility as assigned within scope.

    Education

    • Hawaii State Board of Nursingapproved curriculum for LPN licensure.

    License, Certification,Registration

    • LicensedPractical Nurse (Hawaii) required at hire
    • BasicLife Support from American Heart Association
