    Description

    Responsibilities:

    • Assist residents with daily livingactivities, such as dressing, bathing, andeating
    • Monitor patients' healthstatus and report any changes to the nursingteam
    • Administer medications and treatments asdirected by medical staff
    • Help coordinaterehabilitation programs for patients to aid in theirrecovery
    • Maintain accurate records of patientcare and progress
    • Ensure a safe and comfortableenvironment for residents

    Shift:

    Monday-Fridaywith rotating weekend andholiday

    6am-2pmOR2pm-10pm

    Requirements:

    • Valid NYS LPNLicense
    • Prior experience (6 months) in nursinghome or rehabilitation setting
    • Compassionateand patient-focused attitude
    • Strongcommunication and teamwork skills
    • Ability tohandle challenging situations with empathy andprofessionalism
