    Description
    We are looking for a ConsultingRegistered Dietitian who will provide clinical support at along-term care home.

    Thisposition is for coverage for 5 days perweek, between April 24th and May 26th witha potential to be extended to August 9th.

    KeyResponsibilities

    • TheRegistered Dietitian is responsible for providing residentnutrition/hydration care and documenting nutrition/hydration carein compliance with regulatory standards as per LTC legislation andDietitians ofCanada
    • Toassess the nutrition status for all residents on admission,annually and whenever there is a significant change instatus
    • Todetermine each resident s level of nutrition and hydration risk inorder to prescribe/recommend needed interventions and follow-upcare
    • Todevelop a nutrition plan of care for each newly admittedresident
    • Toevaluate and modify, as needed, the nutrition plan of care forresidents being followed by the RD, on an ongoingbasis
    • Todocument resident nutrition care for residents followed by theRD
    • Toprovide nutrition/dietary counseling/education for residents and/ortheir caregivers, family members or substitute decision makers forthe purpose of supporting optimum resident nutritioncare
    • Tomonitor residents at different risk levels, especially those athigh nutrition risk and are considered by the RD to require ongoingnutritional monitoring. Assessments delegated to the Food ServiceManager are monitored and followed up on a timelymanner
    • Toparticipate as a member of the interdisciplinary resident care teamat resident care conferences, as schedulingallows
    • Toparticipate as a key member of the home s Care Teams andinterdisciplinarycommittees
    • Tocoordinate continuous quality improvement of the home s nutritionand hydration care and food services and dining in collaborationwith the Food ServiceManager
    • Toprovide nutrition/dietary in-service education for Dietary andNursing staff and other interdisciplinary team members asrequired
    • Toreview and approve the home s seasonal menus, including those fortherapeutic and texture modified diets and snackmenu


    Requirements

    • Preferredcandidate will have clinical experience in LTCsetting
    • Atleast 1 year experience in Long Term Care working independently asa clinicaldietitian
    • Currentregistration in the College of Dietitians of Ontario and possessLiability Insurance as per CDOrequirement
    • Mustpossess strong written and oral communication, critical thinking,care coordination, time management and resident focusskills
    • Musthave proficient computer skills using Microsoft Office applicationsand Point ClickCare
    • Successfulcandidate must be willing to support the LTC home during thispandemicresponse
